Urgent Care Social Enterprise keeps older Londoners out of hospital and in their own homes where this is safe and sensible. We are working in North East, South East and South London in partnership with other providers – not in competition.
Our services include:
- Rapid Response Urgent Care Teams
- Mobile Long Term Conditions case management and rapid response for patients with COPD, Heart Failure and Diabetes
- Staffing and Support for Urgent Care Centres
- Targeted support for Nursing & Residential homes especially out of hours and at weekends
